Referrals, Fees & Prescriptions

Appointments
Appointments are available on weekdays and evenings. Please arrive about 20 minutes early for your appointment to complete necessary registration paperwork. Be sure to bring your insurance information with you. If you are unable to keep your appointment we ask that you call within 24 hours to cancel or reschedule.

Prescriptions
Requests for prescriptions are handled only during regular office hours. If you need to obtain a prescription refill please call our office and leave your request on our pharmacy line. We will do our best to call the refill in to your pharmacy by the end of office hours each day. YOUR PHARMACY CAN FAX A REFILL

Fees & Referrals
Payment is required at the time of service. Patients with insurance are asked to pay co-payments, co-insurance, deductibles and charges for non-covered services at the time of their visit. Many managed care plans require written referral or pre-authorization for surgery or other procedures. Our staff can help you with your plan requirements.
